GJK'S PARENT PORTAL 


Welcome to the GJK Parent Portal, where you can access and complete the majority of our school year paperwork.

In addition to completing the information on this portal, please submit a paper copy of the following documents to the Director prior to the start of the school year:
  • Birth certificate
  • Current physical with parent section completed and signed. You may use the State of Illinois Certificate of child Health Exam (link below) or your pediatrician's pre-printed form. 
    • If the doctor is submitting the forms directly to us, the parent will need to complete and submit a separate form with the parent section completed.  The Tb and Lead test sections must be completed by the physician. Vision and Hearing screening is required for all children three and over.  GJK will provide these screenings in the fall for a nominal fee.

  • Completed allergy intake form (provided via email)

Instructions for submitting paperwork:

  1. Create a username and password for your family.
  2. Complete all fields relevant to your child.
  3. When asked choose either school year OR summer.
  4. Each child will have their own set of documents within your family's account. 
  5. This form is a legal document. Enter your full name in the electronic signature field at the bottom. This is a legal signature. Enter the current date.
  6. Click the Save Progress button at the bottom if you have partially completed the form to save all information entered and want to return later to finish
  7. After entering all information appropriate for your child, you MUST click the Submit button for the document to be submitted to GJK
  8. If you want to register another child, sign in again with the family username and password and click the Start New button. Follow the instructions above.
  9. If you want to update information or register your child for another year, sign in again and click the Edit button on the row with the child's name. Make any changes required. Update the date field next to the signature field. Click the Submit button.
  10. There is a password protected link required to access the the GJK parent handbook.  See the email sent with the password or email [email protected].
